Please read the waiver and complete the information as requested. Ensure you check the box agreeing to the waiver AND type your name in the box.  Be sure to provide address information that covers at least the previous five years.  Once you submit the information, our team will complete the background check and provide the results to your organization.

FBPI will notify your organization whether or not derogatory information was found in your background. Please understand that FBPI is not the determining organization. We DO NOT decide whether or not you “PASS” a background check. We are only the reporting agency. 

Checking the box indicates your authorization to use your date of birth (DoB) and Social Security Number (SSN) to conduct a background investigation and check your criminal history, court records and other public and private information. You are hereby authorizing Fort Bend Private Investigators to release the relevant results of this check, including any derogatory information to the organization(s) you’ve indicated.
